Mid-America Christian University Athletic Training
Welcome to Mid-America Christian University! The primary athletic training room is located on the northwest end of the O'Brien Training Center as well as a secondary room located in the Gaulke Activity Center on the west side of the building near the locker rooms. We welcome visiting athletic trainers and will do our best to accommodate your needs. Our facilities are available two hours prior to and thirty minutes after competitions. Heat, ice, electrical stimulation, ultrasound, and whirlpools are available for your use. Please let us know if there will be a special assistance you will need.

Below are the forms necessary for completion and use by the MACU Athletic Training Staff.
- The MACU Sports Medicine Insurance Packet contains forms and policies regarding MACU Athletic Training, the participation in athletic contests and practices for MACU, insurance policies and claim procedures and more. Please fill it out completely and bring it with you when you come to campus for the start of the season.
- The Physical Form within the packet is required for completion of the pre-participation physical that each student athlete must have each year. The first page must be filled out prior to the physical and both pages must be brought to the physical examination, after which they will be turned into the MACU Athletic Training staff.
NOTE: If a student-athlete wishes to have their physical completed by their own primary care physician prior to their arrival on campus, this may be done as long as the completed Physical Form (both pages) is brought to the MACU Athletic Training staff prior to a student-athlete participating in any practices or contests.
- MACU does offer athletic insurance policies to all students through Bob McCloskey Insurance. If a student-athlete needs or wishes to purchase insurance from Bob McCloskey Insurance through MACU, please fill out the packet and bring it with you to the MACU Athletic Training Office. If insurance through Bob McCloskey is purchased, the amount listed on the form for the sport(s) in which the student-athlete is participating will be billed on the student's MACU school account.
- If you have health insurance and do not wish to purchase Bob McCloskey Insurance through MACU, please bring your insurance cards to the MACU Athletic Training staff for copies to be made and kept on file.
